Coconino County Health & Human Services seeking Case Worker — Health Start Program

The Health Start Program is an evidence-based home visiting program that provides home-based education, support, and advocacy services to pregnant/postpartum women and their families in the northern region of Coconino County. The Health Start Program team consists of a Program Coordinator, Case Worker, and a part-time Case Worker. Case Workers carry a caseload of 10-45 families and conduct weekly in person home visits and are required to cover the front desk as needed.
