Survey Available for Bureau of Women’s and Children’s Health Needs Assessment

The Bureau of Women’s and Children’s Health are working on their 5-year Needs Assessment for the Maternal and Child Health Title V Block Grant.  For the needs assessment they are gathering data regarding the health and well-being of women and children throughout Arizona.

Eventually, they will narrow down their list to up to 10 health priorities to focus on over the next 5 years.  As part of the process, they have created a survey to gather the opinions of what their priorities should be, what they are doing well, and where they can use some improvement.

This survey is intended for everyone, you all, partners, moms, dads, neighbors, basically anyone living in Arizona is invited (and encouraged!) to take the survey to share their opinions.

After you submit your responses, you will have the opportunity to provide your email information if you are interested in attending one of their community forums where they discuss the data and work to prioritize needs.  Providing your email is completely OPTIONAL and your email will NOT be connected to your survey responses.
