The Health Start Program is an evidence-based home visiting program that provides home-based education, support, and advocacy services to pregnant/postpartum women and their families in the northern region of Coconino County. The Health Start Program team consists of a Program Coordinator, Case Worker, and a part-time Case Worker. Case Workers carry a caseload of 10-45 families and conduct weekly in person home visits and are required to cover the front desk as needed.
Under general supervision, performs work of moderate difficulty in social services case management of low-income clients; performs related work as assigned.
