The Coconino County Health and Human Services is hiring an Opioid Crisis Response Community Health Worker.

Note: This position is grant funded through Opioid Crisis Response. 

Typical Duties

(Illustrative Only)

  • Interviews and elicits pertinent information from program participants/clients/applicants
  • Provides referrals, information and education to clients; facilitates Wellness Recovery Action Plans (WRAP) with clients
  • Supports clients in treatment, recovery, and harm reduction goals
  • Provides follow-up services and documents interactions with both clients and partner agencies
  • Coordinates services with medical and behavioral health agencies
  • Provides referrals to social service agencies and services
  • Conducts outreach and promotes OCR program services within the community
  • Conducts home visits
  • Compiles data to prepare records and reports
