The Arizona Coalition for Insurance Parity is a collaborative effort of the Arizona Council of Human Service Providers, JEM Foundation, Mental Health America of Arizona, MIKID, and the Neighbors Council.  Our mission is to ensure Arizonans are able to easily access appropriate and timely mental health and substance use disorder treatment through their medical insurance.  We need your help!
All health care insurance companies are required to offer mental health and substance use disorder treatment to you through the 2008 Mental Health Parity and Addiction Equity Act and as one of the 10 essential health benefits under the Affordable Care Act.  We are hearing that this is not always happening.  Sometimes people are denied care, are denied certain types of care, can’t find a provider near where they live, or have to fail with one treatment or medication before a more effective one is authorized.  This can have devastating effects on the individual denied care and their families.
Please fill out this survey if you, or someone you know, has been unable to access needed behavioral health services, had to go “out of network” for care, or had a claim denied for mental health and/or substance use disorder treatment. The information you provide will help us to shape public policy and influence legislation in order to move us towards full parity implementation and mental health equity.
By completing this survey you are acknowledging that your information is being collected to help understand the current trends and circumstances surrounding mental health and substance use disorder treatment claim denials in Arizona. All information you provide will be kept confidential, and any identifiable information you choose to provide will not be shared without your permission.
